Chronic Kidney disease (CKD) affects an estimated 37 million adult Americans and 90% don't know they have it. CKD is a progressive condition characterized by the gradual loss of kidney function over time. Patients with CKD are at an increased risk of cardiovascular disease, anemia, mineral bone disorders, and other complications. Unfortunately, CKD is often asymptomatic until very late stages in the disease or other complications are uncovered. The late diagnosis makes it challenging to accurately estimate the true prevalence and burden of the condition. Also, CKD is often presented with multiple comorbidities, which results in making the condition increasingly complex and difficult to identify.

ROUTINE LABORATORY MONITORING

Routine laboratory monitoring is essential to identify CKD and prevent further disease progression. Rhodes Group has developed a CKD Dashboard tool for New Mexico providers. The Dashboard addresses the challenges of monitoring and identifying the disease in near real-time. Programmed by using national guidelines, it can identify:

+ Patients with CKD
+ Stage of severity
+ Gaps in care
+ Risk factors

This crucial information assists providers in earlier identification so they can refer patients to nephrologists across health systems. Our vision is to leverage laboratory domain knowledge to promote quality of life, care coordination, and health equity. All while minimizing health disparities across a fragmented health system network.
